Print a letter of recommendation form (university students only). CHECKLIST: (1) Application form (2) Release form (3) Recommendation form (for undergraduate level university students ONLY) (4) Copy of passport. (5) Bank deposit receipt All above-listed application requirements (1-5) must be sent by either FAX (011-52-473-102-97) or by a mail service, such as Federal Express, UPS, or DHL to ensure arrival.
